Retinal projection refers to the path of light as it travels from a point in the visual field to a point on the patient's retina. You may remember these graphics from Module 11. As pictured below, an object in the superior visual field projects to the lower half of the retina. An object from the inferior visual field projects to the upper half of the retina.
Therefore, if one eye is pointing upward in relation to the fixing eye, then the image of the object of regard will be projected onto the upper retina of the deviating eye, creating the illusion that there is one object above the other.
In the illustration below, the right eye is fixing and the left eye has a hyper-deviation. The VMR is in front of the right eye and the horizontal red line appears to be "straight ahead". The image of the dot projects onto the superior retina of the left eye, so the image of the dot appears to be below the red line image.
